2. Phone 07766 314 079 to make an appointment for a free consultation at your home.  (you may need to leave a voicemail message if I am out on a pet assignment but I will return your call within 24 hours)

The First Meeting

I understand that it is a big decision letting someone new into your home, for this reason Tail Blazers offers a free initial consultation. During this visit we can all get to know and trust one another and we can discuss any concerns that you may have. I will take notes on diet, medication and any special routines that need to be followed so that you can leave your pet in my care without worry.

At this consultation I will fill in a Registration Request/ Questionnaire to make certain I have all the information I need to provide the best service I can and ensure the utmost safety of your pets.

At this first meeting please have any essential information you would like for me to have including emergency contact details, your Veterinarian information, current vaccination record, and any keys necessary to gain access to service your pets. There is a simple service agreement to sign before services begin.

*Please note that keys to your home are never identified by your name or address and are never stored with your file or personal information.

Insurance
Tail Blazers holds Public Liability Insurance for Property in Tail Blazers custody or control. It is the pet owner’s responsibility to ensure that adequate pet insurance is in place for your pet(s). Tail Blazers will not be held responsible for any accident, loss or injury caused while in our care.
Illness or Injury
If your pet falls ill or is injured during the time it is under the care of Tail Blazers, I will provide high quality of care and all attempts will be made to contact you via the emergency numbers you have provided. If I am of the opinion that the animal is in need of veterinarian assistance your veterinarian will be consulted in the first instance or another veterinarian should yours be unavailable. Tail Blazers will not be responsible for any costs incurred as a result of the services provided by any veterinarian. By registering for service with Tail Blazers the pet owner agrees to pay any costs associated (including Veterinary and transportation costs) with any illness or injury which occurred while under the care of Tail Blazers.

*It is the owner’s responsibility to disclose any relevant and pertinent medical history of the animal to Tail Blazers at the earliest they are made aware of any condition.

Notify Your Vet

I request that you notify your Veterinarian that Tail Blazers will be caring for your pet and that any consultations and/or treatments required for your pets will be paid in full by the pet owner.
